This service is a face-to-face home visit for people who are living in The Southern Highlands, Shoalhaven & Illawarra in NSW.

Support that meets you where you are—literally and emotionally.

During this in-home visit, I come to you to provide warm, practical, and hands-on support. Together, we’ll explore your space, your concerns, and your goals to create a comprehensive wellbeing plan that aligns with your loved one’s needs and your family’s capacity.

We will also assess your home environment to ensure it is safe and/or dementia friendly.

This visit is ideal for those wanting a more immersive and observational approach to support. 

You’ll receive:

• A detailed, personalised plan tailored to your environment and situation

• Education on dementia, fall prevention, and meaningful engagement

• Home safety and routine suggestions to reduce confusion and stress

• Practical tools and strategies that you can begin using right away
