  • Home safety assessment

  • Support in setting boundaries and asking for help

  • Education to help you better understand dementia and ageing

  • Gentle routines for more ease and connection

You’ll also receive helpful resources, and continued support as you apply the plan. You don’t have to carry it all alone—this plan is here to hold you while you hold others.
